Where do I obtain a copy of an Indiana birth or death certificate?

You may obtain a copy by visting the County Health Department where the birth or death occurred. (Check here to find the county office locations).  The Indiana Department of Health no longer provides "walk-in" service as of 12/10/10.  You may order via "VitalChek or for additional information or questions contact ISDH, Vital Records @ 317-233-2700.

If you need a copy for identification purposes (out of country travel) you must have the birth certificate authenticated ( an Apostille Document) by the Secretary of State Office, (317) 232-6531.

 For American Citizens born out of the U.S., you will need to contact Bureau of Consulate Affairs.

To obtain or locate a death or birth records from any individual born in Indiana before 1907 contact  Marion County Vital Records @ 317-221-2000.
