As an adopted child, how do I get information about my birth parents?

Information about birth parents can be obtained from the Indiana State Department of Health, mostly about medical history.

Indiana initiated an Adoption History Program in 1988 (IC 31-19-18 ) to allow for the release of medical, non-identifying, and identifying information. Identifying information can be released only when both the adult adoptee and the birth parent register with the Indiana Adoption History Program. To register, please call 317.233.7279 or e-mail Darci Johnson, Adoptions Clerk - darjohnson@isdh.in.gov

Forms: Adoption (http://www.in.gov/isdh/19042.htm#VitalRecords)
