If you have received an administrative action notice from this office please review carefully. If you wish to have a hearing on a matter, it is critical that you request the hearing within the appropriate amount of time. The hearing should be requested in writing to Judy Smith at firstname.lastname@example.org or 302 W. Washington St., Room E -111, Indianapolis, IN 46204. A hearing is a formal proceeding with a hearing officer and court reporter. Both the applicant and the Division will be asked to present their case, which may include witnesses and evidence. You are not required to have an attorney, but you are encouraged to seek legal counsel. This is not a criminal proceeding so an attorney will not be appointed to you.
